The 'Clinical Intent' Mandate: How New e-RS Updates Impact Your A&G Pathways

Who should read this
IT Managers, Digital Transformation Specialists, Clinical Directors, Service Managers, and Pathway Coordinators.
Summary
NHS Digital has shifted how Advice & Guidance (A&G) is tracked. Recent updates to the e-Referral Service (e-RS) API now require the capture of a referrer's standardised "Clinical Intent" whenever an A&G request is made. The goal is to give Integrated Care Boards (ICBs) complete visibility into non-admitted care activity.
If your specialist departments are still managing A&G via unstructured emails, phone calls, or disjointed legacy forms, you are now operating outside of the latest national data standards. Read on to discover what these four new Clinical Intent categories mean for your hospital, and how the Synergos suite automatically aligns your digital front door with NHS Digital’s latest API standards—saving your IT teams months of development time while securing your pathway funding.

The Evolution of Advice & Guidance Data As Advice & Guidance (A&G) becomes mandatory across elective specialties, NHS England and local ICBs are demanding deeper insights into why these requests are being made. It is no longer enough to simply count the total number of A&G interactions; commissioners want to understand the exact nature of the non-admitted clinical workload.
To facilitate this, NHS Digital has introduced significant updates to the e-RS APIs, introducing four standardised categories of "Clinical Intent." When a primary care clinician submits an A&G request, they must now categorise their intent into one of the following:
Supporting primary care management: Seeking specialist advice to keep the patient safely in the community.
Determining if a referral is necessary: Using A&G as a direct triage step before triggering an 18-week Referral to Treatment (RTT) clock.
Interim care while awaiting appointments: Seeking advice to manage a patient whose condition has worsened while sitting on a long elective waiting list.
Advice for existing hospital patients: Streamlining communication for patients already known to a specific secondary care department.
The Threat of Unstructured Pathways This API update sounds like a minor technicality, but it represents a massive operational headache for Trusts that have not fully digitised their referral pathways.
If a Trust is relying on legacy systems, generic departmental NHSmail inboxes, or basic web forms to process A&G, it is impossible to accurately capture, track, and report on these four specific Clinical Intent categories. Without this metadata, Trusts cannot provide ICBs with the granular data now required to prove pathway efficiency, potentially jeopardising their Aligned Payment and Incentives (API) funding blocks for non-admitted care.
Future-Proofing with referapatient and the Synergos Suite Trust IT and Digital Transformation teams are already stretched to breaking point. Asking them to manually rebuild local A&G forms to align with the new e-RS API standards is an unnecessary drain on resources. The Synergos suite is engineered to solve this exact problem out of the box.
Seamless e-RS Alignment: referapatient acts as your operational Single Point of Access (SPoA) engine. The platform’s digital forms and triage algorithms are continuously updated to reflect national standards. When a GP submits an A&G request through the platform, the Clinical Intent is captured as a mandatory, coded data point from the very first click.
Intelligent Triage Routing: Because referapatient captures the Clinical Intent upfront, it allows secondary care teams to route the request more effectively. An "interim care" request for a patient already on the waiting list can be routed directly to their named consultant, while a "determining if a referral is necessary" request can be routed to the daily triage pool for rapid conversion into a physical clinic slot.
Advanced Clinical Coding: For Trusts utilising our Enterprise model, this Clinical Intent data is married with advanced metadata tagging (such as SNOMED CT, ICD-10, and RxNORM). This provides hospital Service Managers and Finance Directors with the ultimate, audit-ready dashboard, proving full alignment with national e-RS API standards and guaranteeing that all non-admitted clinical activity is accurately recorded and funded.
The Verdict
Data is the currency of the modern NHS. As NHS Digital enhances the e-RS infrastructure to demand greater granularity, acute providers must upgrade their intake systems to match. By positioning referapatient as your digital triage engine, your Trust guarantees compliance with the latest Clinical Intent mandates, eliminating the administrative burden on your IT team and providing your clinicians with the structured data they need to make rapid, safe decisions.
